Enrollment forms
Contact Mill Creek or Hamilton Mill for the latest enrollment packet, required documents and instructions for returning completed forms. Please do not send sensitive child or family information through an unapproved online form.
Medication and health forms
If your child needs medication or individual health arrangements at school, ask the campus team for the current authorization forms and instructions before their first day.
